Effective September 1, 2012, an additional $30 premium is added to your medical plan cost if you or spouse covered as a dependent on your medical plan uses tobacco products. The maximum charge for tobacco use is $30/month added to your health premium. Life insurance and long-term disability premiums are also higher for tobacco users.
If an employee or covered spouse identified as a tobacco-user becomes a non-tobacco user for three consecutive months, the tobacco use status can be updated through Workday. Please follow the instructions outlined in the Workday job aid for Change Benefits. Choose “tobacco use change” as the Benefit Event Type.
