Orientation Forms

Forms provided here are to be completed during employee online orientation. The online orientation includes a checklist to track completion of required forms and serves as an orientation guide. Access to the online orientation is from the training page of the TEES Personnel Services website.

  • Employee Personal Data - Required of all new and transfer employees - may also be used for current employees to document change in personal data such as address or name.
  • W-4 Form - Allows TEES to withhold the correct amount of federal income tax from an employee's pay. Form W-4 changes may also be made via HRConnect in Single Sign On via the "Payroll Data" tab.
  • Direct Deposit - This form is required for your paycheck to be deposited directly in the bank account designated.
  • Statement of Previous State Employment - Verifies prior employment with The Texas A&M University System and any other agency or institution of the State of Texas, including employment as a student worker.
  • Selective Service Registration Status - Required of male employees upon employment.
  • GLACIER Instructions for Foreign National Employees - GLACIER is a nonresident alien tax compliance system designed to allow institutions to efficiently and effectively collect information, make tax residency and treaty determinations, manage paperwork, maintain data, and file reporting statements with the IRS.

  • Required Information Acknowledgement - Provides a list of important policies and procedures as covered in orientation. Signature attests to the fact that the employee has been provided access to, and reviewed, the information.
  • Intellectual Property Agreement - Form required of all employees stating the Texas A&M University System has property rights to intellectual property.
  • Notice to Employees of Workers' Compensation Insurance - acknowledges receipt of the notice that TAMUS provides Workers' Compensation Insurance coverage.
  • External Employment Acknowledgement - Agreement that full-time System employees are expected to devote their time to assigned duties and responsibilities on a full-time basis.
  • Benefit Acknowledgement - Acknowledges benefit and insurance information was provided as part of orientation.
